Targeted Case Management can be defined as locating, obtaining,
coordinating, and monitoring social, habilitative, and medical
services as well as other services related to maintaining the
person’s health, safety, and well-being in the community.
Targeted Case Management is a service that assists individuals
diagnosed with intellectual and/or development disabilities (I/DD).
The individual’s diagnosis must be manifested and documented
prior to the age of 22 in order to meet eligibility requirements
for this service.
Targeted Case Management is a service available throughout North
Carolina that assists people in gaining access to needed medical,
behavioral, social, and other services.
Targeted Case Management is individualized, person-centered, empowering,
comprehensive, strengths-based, and outcome-focused. Targeted
Case Managers are responsible for ongoing monitoring of the provision
of services included in the individual’s Person-Centered
Plan (PCP).
Targeted Case Managers initiate and oversee the process of assessment
and reassessment of the individual’s level of care and the
review of Plans of Care.

A Targeted Case Manager is someone who is familiar with community resources. He/she keeps updated on local, state and federal guidelines.
